COVID-19 Appointment Protocol
The following is a list of steps that must be taken during each in-person visit at Elora Physiotherapy.
1. Covid Screening
On each day of your appointment(s), you are required to fill out an online COVID screening questionnaire. The form will be emailed to you two hours before your appointment. This form ensures you are an appropriate candidate for in-person care. Please note this form must be completed before each appointment. If you do not see this form in your inbox the day of your appointment, please check your junk folder and touch base with us if you do not see it. Alternatively, the screen can be completed verbally at the clinic. If you do not pass the COVID screen, you will not be permitted to enter the clinic and we can utilize virtual appointments instead.
All clients are required to wear a face mask. You may bring your own face mask, which can be either cloth or paper. If you do not have a mask we will provide you one.
If you require someone to accompany you to your appointment, they will be required to provide their name, phone number, wear a mask, hand sanitize, verbally complete the COVID screen questionnaire upon arrival.
4. Hand Sanitizing
Once you enter the building, you must stop and sanitize your hands prior to entering the elevator or using the stairs. Once on second floor, you must stop and sanitize your hands again just outside the clinic door.
5. Waiting Room
Prior to entering the clinic, please remove all footwear and leave on the trays provided outside the clinic. Upon entering the clinic, you may sit in our socially distanced waiting room and we will promptly bring you to your room.
Washrooms are available for use and are sanitized after each use.
All appointments are completed in completely private treatment rooms. If the gym is to be utilized it will be with only 1-2 clients and therapists at a time to maintain social distancing. All equipment and treatment rooms are thoroughly sanitized after use.
The front desk and any touch points are thoroughly sanitized between each client.
As you exit the clinic please sanitize your hands outside the clinic door prior to entering the elevator or staircase, and again at rear of building as you exit.
